New Client Forms

Please make sure to fill out all of the forms below before your first appointment. You can either email the filled out forms to spowell@awarenesscounselingagency.com or print them out and bring them in. This will save you time. If you are a client participating in our 12-step program for rehabilitation we need you to bring your Certificate of Completion form from your agency along with a picture ID.
